2008 CC7D Filmmakers' Documents
Updated: 7/22/08

Below are links to documents that filmmakers will turn in with brief descriptions of each. One hard copy of each document will be provided to you at the Kickoff Event.

Participant's Agreement
The Team Leader must sign this form to indicate agreement to the CC7D rules and requirements, and then bring it to the Kickoff.

The following nine (9) documents will be given to you at the Kickoff Event and are to be turned in at the Wrap-Up Party.

2008 Team Checklist
Here is a checklist to help you keep track of all your paperwork. It lists all of the documents you are to turn in with your film on July 31.

2008 Wrap-Up Form
This document assists CC7D by gathering basic information about your film (running time, etc.), equipment and your thoughts/suggestions on changes for future competitions.

2008 CC7D Certification
The Team Leader must sign this form to certify that all creative work took place during the Official 7-Day Filmmaking competition period.

2008 Team Roster
This document succinctly lists everyone involved in the creation of your film. If your cast and crew list can not fit into the space provided, please use duplicate sheets.

Liability Waiver Form
This document is a limited liability form that protects you and CC7D. All cast and crew must sign. Each team member must sign a separate form.

Talent Release Form
This document must be signed by everyone who appears onscreen in your film. This insures that you have been authorized by everyone in your film to distribute, screen, show and/or sell their image. You will need to make one copy per person. This includes people who may incidentally appear in the background of your film. In other words, if you shoot a scene at the mall or in a bar, everyone who appears on screen must sign this form.

Music Release Form
It is illegal for CC7D to screen films that include unauthorized, copywrited music. This document proves that you have obtained the rights to use all of the music that is in your film. If you are purchasing royalty-free music, please provide a receipt of the purchase (this is usually an online purchase) with any personal information removed (credit card number, home address, etc.), along with an explanation on this document. If you use music from a royalty free music CD, please provide a copy of the contract for use of reflecting the CD as well as a copy of the CD cover along with an explanation on this document. If you are using any music not found at a royalty-free website or from a royalty-free album, this music release form must be signed by the appropriate parties. CC7D will err on the side of caution when it comes to the use of unauthorized music in CC7D films.

Materials Release Form
This document is to be used for photographs and other materials.

Location Release Form
This document proves that you have obtained permission to shoot on private property. Exteriors of buildings, if shot on public property, do not require this document.
